Birth & Death Certificates Office of Vital Records The Collier County Health Department serves as the county's vital records office for birth and death certificates. The Health Department can issue birth certificates for the years 1930 to the present for births within the State of Florida. Death certificates are issued by the Health Department for the year 2007 to the present for those individuals who passed away in Collier County only. Death certificates for other years or counties must be requested from Jacksonville. Death certificates, with cause of death, can be issued to immediate family members of the deceased or to a licensed funeral director. Birth certificates are available from the Collier County Health Department for ALL Florida counties. Birth Certificates are needed for:
Birth certificates can be issued to parents, to an individual 18 years of age or older and, with proper identification, to legal guardians with proof of legal custody. Types of identification accepted are: state I.D. card, driver's license, passport, or military I.D. To request a birth or death record from the State Office in Jacksonville, phone (904) 359-6900 ext. 9000 or send a written request to: State Office of Vital Statistics Birth certificates issued by the Health Department cost $10.00 for all Florida Births. A second certificate costs $6.00 if issued at the same time as the first certificate. Death certificates issued by the Health Department cost $8.00 each.
