Death Certificates

Application for Certified Copy of Death Certificate:

Cause of death is confidential information and can only be issued to immediate family members, which FS 382.025 defines as "spouse or parent, or to the registrant's child, grandchild, or sibling, if of legal age, or to any family member who provides a will, insurance policy, or other document that demonstrates the family member' s interest in the estate, or to any person who provides documentation that he or she is acting on behalf of them". Cause of death cannot be shown for anything that will be filed as a matter of public record, i.e., real estate transfers, automobile transfers, recording of wills, etc.

On-line applications are not currently being accepted. Please print out the above information and mail in with payment of $8.00 for each certificate, as well as a self-addressed, stamped envelope (legal-sized if ordering more than 5 certified copies), and mail to:
