School Health Program
Services:
PARTNERSHIP
The Collier County School Health Program operates through a partnership between Collier County Public Schools, Naples Community Hospital and Collier County Health Department. Each entity has legal and contributory roles and responsibilities, which address the needs of Collier County school children.
A two-year school health services plan is created through the partnership interactions with the School Health Advisory Committee. A current Health Services Plan is available. It provides statistical and other program information covering the various levels of services provided by the Partnership in Collier County schools.
GENERAL BASIC SERVICES
School heath services are provided to all Collier County public schools. Private schools receive basic services, upon request, including the screening of students for hearing, vision, growth, development and scoliosis. Each student whose re-screening results continue to indicate the possibility of a need, are referred for physician and/or other evaluations. The Health Department follows the referral outcomes. Assistance is available for those families who experience financial or other difficulty in obtaining vision follow-up and glasses.
SCHOOL NURSING SERVICES The School Health Partnership provides nursing
services for Collier County public schools. Nursing staff provide nursing evaluation, care, case management and health education in accordance with the Florida Nurse Practice Act and local policies.
SCHOOL ENTRY HEALTH REQUIREMENTS
1. Physical Exam: Students new to Florida Schools are required to have a physical exam from a licensed health care provider on file at the school. Typically, the exam will be documented on a Physical Exam Form 3040. 2. Immunization status must be up-to-date. This means that all immunizations, both in number and timing, according to the child's age, must be documented and on file at the school. Rare exceptions to this occur. Contact with the Collier County Health Department Immunization staff is needed for any non-medically based exceptions. Please refer to the immunization section of the Web site.
